I worked as a Physician Assistant in the ER for years before I retired from that. It was both rewarding and frustrating at the same time. Of course, the rewards come in saving a life or diagnosing a condition that had gone undiagnosed for years by all the ‘experts’ – and I was not even a doctor, just a Physician Assistant. But it was so frustrating to see many people come into the ER who NEVER NEEDED TO BE THERE!
They had no where else to go and so they came to us.
Then, in order to make sure we didn’t miss anything, WE WOULD ORDER JUST ABOUT EVERY PLAUSIBLE RELATED TEST – and no one questioned it. So much waste. In fact, it was estimated that 50% to even 70% of ER visits are not needed – WASTE! And then all the wasted tests we would do!
What’s worse, is that the hospital would strongly encourage us to admit patients so that the hospital could get the revenue. There was even one doctor who was fired because this doctor was not admitting enough patients!
And there is more. When the patient was going to leave, I would sit down with them and their family and print off all kinds of educational material and explain it to them in detail, so that they could learn and not have to come back. The patients and the nurses constantly told me that I was the only one who did that. Again, how frustrating that I was the one of the only ones trying to educate the patients. The doctors and other physician assistants would tell me not to waste my time, “It doesn’t do any good, and besides who’s got the time?” they would all say.
When I was in my twenties, and was in school, I would volunteer at Free Clinics around the country. I worked at the Cleveland Free Medical Clinic, the Haight-Ashbury Free Clinic, and the Washington DC Free Clinic. They all had the right attitude of working with the patients to educate them and take care of them.
I knew back then that I wanted to empower the patients and help them get the means to control their own health and be the healthiest they could be. But that's not where health care is focused today. And the trend is worsening.
Did you know that right now, 1 in 7 people are diagnosed with Type II Diabetes? And it is reported that there is an equal number of people who have diabetes, but they don’t know it! Why? It’s 98% lifestyle! PERIOD. So many of us eat the wrong food, don’t exercise and expect a pill to make us better.
There are patients who believe it's genetic. Most times, it's not. What is passed down is not a gene that causes Type II Diabetes, rather it's a lifestyle that's passed down from one generation to the next that causes the poor diet and sedentary lifestyle that caused the diabetes.
At this rate, by 2050, 1 in 3 people will be diagnosed with Type II Diabetes! And that means that it will probably be 1 in 2 who actually have it. AND IT IS TOTALLY PREVENTABLE! In fact, 80% of ALL diseases are lifestyle based.
Heart Disease and Diabetes CAN BE PREVENTED IN OVER 90% OF THOSE DIAGNOSED WITH IT! But all everyone seems to want to do is take a pill and keep on going down the path of disease, poor health and high costs.
An article came out recently that tracked thousands of obese adults and discovered there was a link between obesity and high blood pressure (Really? They needed a study to know that?). What was frustrating was the conclusion. “According to the researchers, one way for the ... healthcare system to address these risk factors would be the management of high blood pressure with antihypertensive drugs.”
Drugs! That’s their answer! No one even suggested, ‘Hey maybe you might want to try and lose some weight.’
Do you see why I am frustrated?
Now let's address the real costs of all this waste.
We spend about $3 Trillion in healthcare every year and waste OVER $1 Trillion – that waste is more than we spend on our total military (about $700 Billion).
Every year health insurance premiums keep going up and up. And every year people get less and less for it! Deductibles are so high that people don’t go see a doctor at all. And if they want to see a doctor, they oftentimes have to wait a month or two, and when they get in, they have to wait an hour or so in the dirty waiting room, and then another 30 minutes in the exam room and then what…?
…WHOOSH! The Doc whirls in, says hi, looks at your vitals and scans your chart, asks how you are and quickly types stuff in – sometimes never looking at you or examining you – and then scribbles out orders for labs, a script and a consult with an expert and WHOOSH! The Doc’s gone!
There was no discussion on how to get healthier. NONE! And half your day is gone.
And it’s not their fault. Forbes magazine researched and found out that the insurance companies demand that a doctor spend no more than 10 minutes with a patient. Many doctors are required to see 40 to 60 patients a day! (Divide out 60 patients into 8 hours and you get 8 minutes a patient – and when you include the extra stuff the Doctor has to do in between patients, then that comes to about 5 or 6 minutes per patient.
Oftentimes you have to pay $125 for those 5 minutes. Of course, as soon as you leave, you remember a question you wanted to ask, but there IS NO WAY you can get back in to see the Doctor. The Doctor is on to three more people by this time.

This won’t reduce the cost of insurance, but hopefully it will reduce the NEED for insurance. Or at least the deductible is tolerable now. Before a patient had a $6,000 deductible and would never get any help because of all the costs and lost time associated with a ‘simple’ visit. This is an important gap in American Health care that is now fixed for 90% of all diseases.
We’re just right for those who have insurance but are frustrated by the lack of access, or the high cost of the deductible, or the lack of focus on getting healthier. As well, those who have no insurance and need affordable care of a doctor and a plan to get healthier are both great members for us.
